Calcium channel blocker

2.5-5mg IV 30mg

Contraindicated in WPS or sick sinus syndrome.

5. calcium chloride Generic only Hyperkalemia (except with digitalis toxicity), Calcium channel blocker toxicity

Electrolyte

8-16mg/kg slow IV Doctor order varies.

Tissue necrosis followed by IM use or IV infiltration.

6. diltiazem Cardizem A-Fib, A-flutter, Multifocal Atrial SVT

Calcium channel antagonist

0.25mg/kg IV titrate to heart rate

Shelf life at room temp is 1 month.

160-325mg PO/Chew 100mg/kg/day

Contraindicated in children with flulike symptoms.  Overdose ringing in the ears.

8. morphine sulfate Astromorph Chest pain not right sided. pulmonary edema, moderate to severe pain.

Opioid analgesic

2-4mg IV 15mg

Do not use with head injuries.

9. nitroglycerine Nitrostat Ischemic chest pain, CHF hypertension, pulmonary hypertension.  Not given if right ventricular failure

vasodilator

0.3-0.4mg SL 3 doses / 1.2mg total

Do not give if recently taken Cialis, Levitra, Viagra.

10. promethazine Phenergan Nausea, vomiting, motion sickness, pre/post operative, sedation, potentiation of analgesics.

Phenothiazine, antihistamine

Is given with narcotics to potentiate effects.

Contraindicated in torsades.  Takes 10 minutes to start to work.

14. magnesium sulfate Generic only Seizures or eclampsia, torsades, hypomagnesemia, refractory V-fib.  Status asthmaticus not responsive to beta drugs.

Electrolyte, anticonvulsant

1-2g IV 30-40g/day

Contraindicated in heart blocks or myocardial damage. Dilute in D5W.

0.2mg IV over 15 seconds. May increase to 0.3mg then 0.5mg No max dose in emergency

Do not give if seizure is possible.

19. epinephrine 1:10,000 Adrenalin Acute allergic reastion, cardiac arrest, bronchial asthma

Sympathomimetic

Cardiac: 1mg IV every 3-5 min. Alergic reaction = mild 0.3SQ, moderate/severe 0.3 to 0.5mg IV No max dose in emergency

Deactivated by alkaline solutions.  IE Sodium Bicarb

23. digoxin Lanoxin SVT's (especially a-fib/flutter), CHF, cardiogenic shock.

Cardiac Glycoside

10-15mcg/kg IV Maintenance dose as ordered.

Toxicity: Yellow or Green vision. Skin rash.

24. Atropine atropine sulfate Unstable Bradycardia, organophosphate

Anticholinergic agent

cardiac 0.5 to 1mg IV.  Organophosphate poisoning 1-2mg 0.04mg/kg

Paradoxical bradycardia if pushed too slow.

25. isoproterenol Isuprel Hemodynamically significant bradycardias unresponsive to atropine, TCP, dopamine.

Sympathomimetic

1mg in 250mL D5W IV.  Infuse at 2-10 mcg/min until desired heart rate

Best use is for hemodynamically significant bradycardias secondary to beta blocker overdose.  Also helpful in the management of torsades.

May precipitate Wernick’s encephalopathy.

29. naloxone Narcan Reverse CNS/respiratory depression from opioids.

Opioid Antagonist

0.4-2mg IV/IM/SQ 10mg over 10 minutes

Works on morphine, heroin, methadone, oxycodone, codeine, demoral
